Before Your First Visit<\/h3>\n

Paperwork<\/strong><\/p>\n

Please either complete the New Patient paperwork<\/a> or print, fill out, and bring paperwork to your first appointment.\u00a0 If you need to fill it out at the center, please arrive 15 minutes prior to your appointment time.<\/p>\n

Food<\/strong><\/p>\n

Please eat lightly a few hours before treatment \u2013 it is best to have some food in your belly prior to treatment.\u00a0 Part of the diagnosis involves looking at your tongue so if you usually brush your tongue, try to skip it the day of your treatment.\u00a0<\/p>\n

Clothing – Community Acupuncture Patients Only<\/strong><\/p>\n

\u00a0Wear loose clothing.\u00a0 The points we choose are most often located below the elbows, below the knees, and on the head and ears (regardless of where your issue is located).<\/p>\n

At Your First Visit<\/h3>\n

Arrival Time<\/strong><\/p>\n

Please arrive 5 minutes early to your appointment (15 if you need to fill out paperwork).\u00a0<\/p>\n

Wait in the reception area; the acupuncturist will meet you there.<\/p>\n

On Return Visits<\/h3>\n

Community Acupuncture patients<\/u> remove shoes, silence cell phones, and make yourself comfortable in a chair in the treatment room. The acupuncturist will meet you there, ask you questions, and then give you a treatment.\u00a0<\/p>\n

Private treatment patients<\/u> wait in the reception area and the therapist will meet you there.<\/p>\n

Payment Information<\/h3>\n

Community Acupuncture Payment Range<\/strong><\/p>\n

First visit $30-$60, return visit $20-$50.\u00a0 Pay what you can afford within payment range; no questions asked.<\/p>\n

Private Treatments<\/strong><\/p>\n

All private treatments are on a set-fee schedule.<\/p>\n

We accept cash (sorry, no change available), checks, as well as Visa, MC, Discover, American Express, and Apple Pay.\u00a0<\/p>\n

Insurance Information<\/strong><\/p>\n

We do not bill insurance directly.\u00a0 Therefore, we are considered an out-of-network provider. \u00a0If you have insurance that covers acupuncture, you can request a billing statement from us to submit to your insurance company for reimbursement.\u00a0 Or, you may use a flexible spending or healthcare savings account to pay for your acupuncture treatments.<\/p>\n

Cancellation Policy<\/h3>\n

We request 24 hours notice<\/strong> in advance of an appointment if it is necessary to cancel or reschedule that appointment.\u00a0 You can do so by calling us or changing your appointment online (please note \u2013 online requires 24 hours\u2019 notice).\u00a0 All community acupuncture appointments cancelled or rescheduled with less than 24 hours\u2019 notice and appointments missed without notice will be charged a $15 fee; $30 for private treatments.\u00a0 If appointments have been purchased with a gift certificate or as part of a package, missed, cancelled, or appointments rescheduled less than 24 hours in advance will be deducted from the number of remaining appointments.\u00a0 Thank you for your understanding.<\/p>\n

Most patients schedule online<\/a> but you can also call the Clintonville (614) 725-2488 or Westerville (614) 426-4406 location, and someone will assist you with scheduling.\u00a0 If you are new to Urban Acupuncture (even if you have had acupuncture before), please schedule a Consultation and First Treatment appointment (for Community Acupuncture) or a Private Consultation and First Visit for a private session.<\/p>\n

\"CommunityBeing Treated in a Community Acupuncture Setting<\/h3>\n

The treatment room is intended to remain a quiet space for you and others to relax.\u00a0 We appreciate everyone\u2019s presence \u2013 we find this kind of collective stillness a rare and valuable thing in our rushed and isolating society.\u00a0 Maintaining this reservoir of calm requires that no one talk very much in the treatment space or in the reception area.\u00a0 We ask that any speaking be done in a whisper or very low tone of voice.\u00a0 We ask that the lobby also remain a quiet space.\u00a0 Talking is of course necessary and acceptable, but please keep voices as low as possible to allow those still being treated to relax.<\/p>\n

Part of our success relies on our patients learning the \u2018routine\u2019 and taking on responsibility for their appointments.\u00a0 Please try re-scheduling and making payment at the front desk before each treatment, so you can relax and enjoy the treatment without concern for details afterwards.\u00a0 Please remove your shoes and place them in the area provided.\u00a0 Feel free to hang your coat on the rack or take it along with your other personal belongings back into the treatment room (you will find baskets on the floor to place them in).\u00a0 And of course, please mute your cell phone before coming into the treatment room.\u00a0<\/p>\n

Your responsibilities:\u00a0 we do not provide primary care medicine.\u00a0 Acupuncture is a wonderful ally to western medicine, but it is not a substitute for it.\u00a0 If you think you have a potentially serious condition such as an infection, unexplained weight loss or gain, consistent chest pain, a suspected fracture or dislocation, severe sudden abdominal pain, unexplained bleeding, respiratory distress etc. or if you want a doctor to go over the details of your medical history, you should either visit your primary care physician or go to the local emergency room, as appropriate.\u00a0 Please do not expect us to diagnose and treat something life threatening.\u00a0 However, we can provide medical care for many other conditions very effectively.\u00a0<\/p>\n

If you are currently experiencing fever or an active cough, please do not come in to be treated until resolved.<\/p>\n

Veterans<\/h3>\n

Urban Acupuncture Center is a licensed provider with the U.S. Veteran\u2019s Administration for acupuncture and massage therapy.\u00a0 If you are a Veteran, please contact your local VA and ask for a direct referral to Urban Acupuncture Center.\u00a0 Only those veterans with an active referral from the VA are eligible for this benefit.<\/p>\n

Worker’s Compensation<\/h3>\n

Urban Acupuncture Center is an authorized provider with the Ohio Bureau of Workers\u2019 Compensation.\u00a0 If you already have a claim with prior authorization for acupuncture through the OhioBWC, you are eligible for this benefit.<\/p>\n
